A B12 shot is a quick intramuscular injection of vitamin B12 (cobalamin) that bypasses the stomach acid, liver metabolism, and intestinal absorption issues that limit oral B12 to roughly 1% bioavailability in most adults. Why B12 Matters
Vitamin B12 (cobalamin) is essential for energy production, neurological function, red blood cell formation, and methylation — the biochemical process that keeps thousands of pathways in your body working correctly. Most people get enough B12 from their diet, but many have absorption issues that mean adequate intake doesn’t translate into adequate cellular levels.
According to the NIH Office of Dietary Supplements, functional B12 deficiency is most commonly seen in patients with one or more of the following:
- Age over 50 — stomach acid production declines with age, and stomach acid is required to free B12 from food
- PPI or H2 blocker use — reflux medications suppress the acid needed for B12 absorption
- Metformin use, which interferes with B12 absorption directly
- Vegan or vegetarian diets — B12 is found almost exclusively in animal products
- Gut conditions including celiac, IBS, SIBO, and Crohn’s
- MTHFR mutations that affect how B12 is metabolized once absorbed
- Heavy alcohol use
The symptoms of low B12 are easy to dismiss: fatigue, brain fog, low mood, tingling in the hands or feet, weakness, and shortness of breath on mild exertion. Many patients have been carrying these symptoms for years, assuming they’re just part of getting older.
Why Injections Beat Pills (For Most People)
The simple math: injectable B12 bypasses the stomach acid, liver metabolism, and intestinal absorption issues that limit oral B12 to about 1% bioavailability in most adults. The cobalamin enters the bloodstream directly and is available to tissues within hours.
The published research on oral B12 bioavailability consistently shows around 1% absorption — meaning of a 1000mcg oral dose, only about 10mcg actually reaches your cells.
For people with absorption issues (and most adults over 50 have at least some), oral B12 simply doesn’t deliver enough cobalamin to make a felt difference. This is why an oral B12 supplement might do nothing for your fatigue, but a B12 injection makes you feel different by that afternoon.
The Different Forms of B12
Not all B12 is the same. We offer injections in multiple forms because not everyone metabolizes B12 the same way, and the form actually matters more than most patients realize.
Methylcobalamin is the active, methylated form. Preferred for patients with MTHFR mutations (which are common in the general population) and for patients who want the most directly usable form. This is our default at InfusaLounge.
Hydroxocobalamin is a storage form your body converts to active B12 slowly over time. Longer-lasting, sometimes preferred for patients with active detoxification goals.
Cyanocobalamin is the cheapest synthetic form, common in over-the-counter supplements. Triny rarely recommends this one. Your body has to convert it before it can be used, and many of the patients who walk in saying “I’ve taken B12 for years and never felt anything” turn out to have been on cyanocobalamin the whole time.

How Often Should You Get a B12 Shot?
Frequency depends on your baseline B12 status and your goals. For most healthy adults on maintenance, once or twice a month is plenty. For active fatigue or low energy, Triny typically runs weekly injections for 4 to 6 weeks, then tapers to monthly. For diagnosed deficiency, weekly for 6 to 8 weeks, then biweekly, then monthly maintenance based on follow-up labs.
Athletic and high-performance patients usually do weekly during training peaks and monthly during off-cycles. Weight loss patients, especially on GLP-1 therapy, often run weekly for the first 8 weeks then taper to biweekly — MIC Lipo is the most common shot Triny recommends for this purpose.
For patients pairing B12 with NAD+ IV therapy during energy-focused protocols, B12 typically runs weekly between NAD+ sessions and helps sustain the cellular energy benefits between infusions.
Walk-Ins Welcome
B12 injections are one of the few therapies at InfusaLounge that don’t require an appointment for returning patients. If we have a nurse available, you can walk in, get your shot, and be out in five minutes.
Larger stacks (MIC Lipo, glutathione, Tri-Immune) typically take 5 to 10 minutes total. Our front desk will check you in, Triny or one of our nurses will administer the injection, and you’re on your way.
Frequently Asked Questions
How quickly will I feel the B12?
Some patients feel it the same afternoon. Most notice a clear energy boost by the next morning. The cumulative effect builds over 2 to 4 weeks of consistent injections, which is why Triny typically recommends a loading phase before transitioning to monthly maintenance.
Can I get too much B12?
B12 is water-soluble — your body excretes what it doesn’t use. Toxicity is extremely rare. Most adults can safely get weekly injections indefinitely. Dr. Gee provides medical oversight on protocols for patients with complex medical histories where additional monitoring is warranted.
Do I need a blood test first?
Not necessarily. For maintenance or general energy support, no test is required. If you suspect a real deficiency, Triny will recommend a serum B12 and methylmalonic acid (MMA) test to confirm. Both are easy to add to a routine lab order.
Are walk-in B12 shots really walk-in?
Yes. We do ask new patients to fill out brief intake paperwork on their first visit (about 5 minutes). After that, walk-ins are quick — just stop in during business hours.
Can I do B12 shots if I’m pregnant or breastfeeding?
B12 is generally considered safe in pregnancy and breastfeeding, but Triny will review your specific situation at intake. Pregnancy is one of the times your B12 needs actually go up, so many pregnant patients benefit from regular injections under their OB’s coordination.
Can I combine a B12 shot with other InfusaLounge therapies on the same visit?
Yes. B12 is often added to IV therapy visits, and many patients schedule a B12 shot the week between their IV appointments. It combines well with everything we offer.
